How to write a letter to Bank for wrong entry in passbook

Dear Madam,

We regret to bring to your notice that I found two wrong entries in my pass book dated 01 May 2011 & 01 June 2011. We had deposited a cheque valued Rs.10000 for collection and the same has not been credited to our account yet.

There is another debit entry of Rs 4000 still no cheque for the said amount has been issued by us.

Could you please have look into the matter and do the needful at the earliest.
